What an episode with Dr. Reetu Singh who is CMO at Saint Francis Health System, but she also spent years as a CMIO before this - so she thinks about technology, standardization and clinician trust differently than most CMOs. Here were the 9 most interesting things we got into:
1/ Why she thinks most systems have it backwards: the budget should NOT be driving the clinical decision. Instead the clinical outcome and the care delivery model should be driving the budget. Simple to say, very hard to actually run a health system that way.
2/ Where she draws the line between standardization and autonomy: there is no standard patient, so you cannot standardize everything. Her test for which steps get locked down and which get left to the clinician is narrower than what most systems apply.
3/ What she listens for that no dashboard will ever tell her: workarounds. If your staff have quietly built one, the process is already broken, and by the time it surfaces in your metrics it's lagging. You’ll have to listen to hear her follow-up question when someone says "don't worry, I figured it out".
4/ Why she insists on managing change WITH clinicians rather than to them. What looks great in a boardroom often falls apart the moment a clinician has to use it, and she's blunt about who needs to be in the room before you buy or build anything.
5/ How she thinks leaders should walk the conference floor. You don't wander through Home Depot admiring a nice-looking tool and then go find a use for it. Once something is broken, you go to the aisle, you buy what fixes it. So why is it that most digital strategy runs in the the other direction?
6/ What clinician involvement looks like when a system actually means it. She says you would not sit in a meeting at Saint Francis that affects clinicians without multiple clinicians at the table, and their large changes run through a clinician dyad partner in operations.
7/ Why "clinical transformation" might be the most overrated phrase in healthcare. Say it to a room of frontline staff and nobody knows what you're actually fixing. Her alternative is far smaller and more boring than you'd expect.
8/ How she coaches clinicians to handle the patient who shows up with ChatGPT answers: stop fighting it, and point them to the sources you trust instead. Her reasoning has less to do with AI than with how long people are waiting to get an appointment.
9/ What she brought to a finance team town hall to make quality feel real: the story of a giant saddle PE pulled out of a patient who went home the next day. She could see it land, and she's convinced stories move non-clinical teams in a way a KPI dashboard never will.
