@CVCSCCM Exit plan is crucial for the continuum of care. We have recently implemented night interdisciplinary rounding to assist with this and ensure the agreed upon plan from earlier is still occurring and if adjustments need to be made based upon critical changes. #cvcsccmjc

@CVCSCCM Of these options I would say fear of self extubation. This is in relation to low staffing numbers and higher nurse/patient ratios. The other is due to a delay in extubation times and nurses not being able to see the benefits of weaning sedation. #cvcsccmjc
@CVCSCCM As long as the patient is stable and on appropriate ventilator settings, weaning sedation/ SAT should be performed to assist with optimizing care and preventing further complications #cvcsccmjc
