requiring that part of the form be given to non medical personnel is not the best practice. Requiring that whole history page/changing the requirement for menstrual data is not unless they are also requiring ATs or nurses in every high school, who are bound by HIPAA.

FWIW: the Florida high school sports physical form that people are discussing is derived from a form that is established as best practice. It’s purpose is to give providers information that may clue them in on underlying conditions (anemia, PCOS, etc). HOWEVER,
