@juanjo_fraire@zoyaalii_@GLiperis@maclaudiaa Surely that there is the need of well designed large prospective studies but it is biologically plausible that all mechanisms leading to the establishment of a limited pool of primordial follicles during foetal life might also have affected the quality
@zoyaalii_@GLiperis@maclaudiaa There are studies showing that Amh may be an age independent marker of quality. Some articles showed increased risk of miscarriage and Down syndrome in women with low amh despite young age, suggesting reduced quality. #ESHREjc
@maclaudiaa Our role now is to understand how to use this test and how to advise and inform patients with low amh. Before understanding whether or not to propose amh as a screening test we should understand what to do once a condition of poor ovarian reserve is recognized in a young woman
@maclaudiaa AMH test is one of the things that has really changed our practice. 20 years ago there was no attention by gynecologists to the ovarian reserve. The amh test has given us the possibility of estimating ovarian reserve. Although not perfect, this is the best we now have
@juanjo_fraire@klattes@BuratiniJose @Jenny_A_Visser @PolyzosN Prolonged pituitary (Fsh and Lh) suppression would lead to a reduced number of antral follicles hence explaining the reduced serum levels of AMH.
@simoniego Probably increased pollution and exposure to chemicals which may act as endocrine disruptors. Testis are highly sensitive to those molecules
Sperm concentration dropped from 101 million/ml in 1973 to 49 million/ml in 2018: a drop of 51.6%. This decline has been found in all the continents. The rate of decline is increasing and it currently seems to settle at -2.64% per year.
