Postpartum intrusive thoughts are about wild things your brain thinks up because it's got a new protective instinct to protect the baby and it makes up unrealistic scenarios. She didn't have an intrusive thought about actually harming her baby. That's the difference.

Normal mother's have these. Like walking down the stairs and imagining tripping. Things like that.
