๐ Development and validation of SONAR-7: a novel ultrasound score for synovitis
Ultrasound is increasingly central to the evaluation of inflammatory arthritis, but feasibility and standardisation remain key challenges. This study introduces SONAR-7, a concise composite score designed for patients with psoriatic arthritis (PsA) and axial spondyloarthritis (axSpA).

Integrates greyscale and power Doppler findings across 14 joints
Demonstrates high specificity (95.1%) with good overall discrimination (AUC 0.83)
Shows consistent associations with:
Clinical disease activity (e.g. swollen joint count, DAPSA)
Enthesitis burden
Functional outcomes (HAQ, BASMI)

Chondrocalcinosis of the knee ๐
Pearls:
- Fibrocartilage CPPD (in the meniscus) occurs at a younger age and is more prevalent than femoral hyaline cartilage CPPD.
- Chondrocalcinosis in both the fibrocartilage and hyaline cartilage is frequent in patients > 80 years old (47% and 23%, respectively).
What is your age cutoff to screen for secondary causes of CPPD?! --> hypophasatesia, hypomagnesemia, hyperparathyroidism and hemochromatosis

๐ก US abnormalities-especially B-mode+ Doppler signal-were associated with relapse.
๐ง Take-home: ultrasound helps identify patients at risk after treatment tapering,
but therapeutic decisions should not rely on imaging alone.
@dr_immuno29 Very pragmatic.What i find it particulary helpful is the emphasis on avoiding unnessecary biologic switches during pregnancy-prioiritizing disease control over theorietical risks.I was wondering whether pre-conception counseling for male patients, was addressed in the guidelines?
