@orangedis I largely agree with you but not on this point. The most fundamental harm we can do to ourselves is to remain ignorant by not having the courage to look at ourselves honestly. Admonished for raising concerns. No adequate system response to escalation. Exec teams & culture are 🔑

@VivekKhashu @codingbrown @jim_crawfurd The poor unseen patients at home … I was scooping walk ins off the WR floor and carrying out frantic resuscitation attempts. Too often futile because they missed their window of opportunity while waiting for an ambulance at home. Devastating for all involved.

While the introduction of the 4 hour standard in 2004 was revered, times have changed significantly. It doesn’t show where the problem is in the system when it is not met. As you say there are lots of contributing factors outwith ED not measured

@RCEM_VP@RCollEM Of course there are better options but that would require the system response we are all desperate to see. Most Trusts continue to approach the issue as they always have despite college guidance. It takes incredibly strong leadership to do otherwise and turn that tide.
Toxic cultures exploit generosity. Takers impose extra work on givers, and no good deed goes unpunished.
Healthy cultures reward generosity. People protect givers from takers, and cream rises to the top.
Helpfulness is not a weakness to target. It's a strength to safeguard.
@RobJimFleming This is not uncommon in surgery. I saw what my husband endured during his surgical training. He did consider a fellowship (most seem to) but with encouragement he stepped off the treadmill, realised his self worth and self respect. He is now flying in his consultant job.
@ShivaniM_KC following unbelievable MSF feedback I was once told my educational supervisor that no one doubted my clinical ability but that I didn’t have consultant presence. Once I recovered my breath I told him that was his opinion and not fact. Brutal and unhelpful. You can’t change you
@elinlowri Absolutely this. As a relatively new consultant I found the subject of austerity and pay erosion difficult to have with my piers despite raising it. Keeping abreast of pay change is as important as the curriculum changes for example, that were belaboured by comparison.

We had to make 75% of sessions to avoid ARCP ramifications. The threats filled the sessions.
