A compliant medical certificate includes practitioner’s name, registration no, contact details, consultation date, & period of incapacity. It does not have to disclose a diagnosis, “medical condition” is sufficient. Employers cannot require disclosure of a diagnosis.

I agree the certificates give no insight in terms of what’s wrong, making them useless to the commission. But then all sick notes are written like that. I’ve never seen one stating an illness. A medical report does, not a sick note for leave of absence.
