Morals of this story:
1. The UpToDate drug information on warfarin contains a consideration on Olestra (how this rabbit trail started).
2. Olestra is banned in many countries, but apparently can still be found in some food products in the US.
3. Don’t eat Olestra.
While this could pose a problem with warfarin uses, Olestra had other problems as well. I mentioned it inhibited the absorption of vitamins. Turns out it inhibited the absorption of pretty much everything else as well. The result?
Olestra earned itself this warning label:

When considering bridging, FVL can be a big factor, but be sure to assess clot risks based on heterozygous vs homozygous.
The risk of thromboembolism is ~3-8x higher if heterozygous. If homozygous, that risk is ~50-80x higher!

Bioprosthetic heart valves last about 10-15 years, while mechanical last 20-30. On the flip side, chance of clot is roughly 2x higher with mechanical valve.
Coming from a VA PGY1, I saw very few (0) breast feeding mothers as patients. This week I learned that it is safe to breastfeed while taking warfarin!
