@BoneJointOpen@ISHASociety The best I can tell, using the Murphy protocol, but choosing a spot just slightly proximal to line โdโ is the best. You want to be at the most proximal aspect of the lesser trochanter. If you see the medial protrusion of the lesser trochanter you are too distal.

@NuelleSportsMD@MizzouOrtho Iโve had this happen several times. Now I drill the lateral root repair tunnel through the lateral portal and my trans tibial drill guide is positioned over the tibial tubercle so I avoid tunnel convergence
@doctorgelber Had a couple. Requires evaluation of the trochlea to help determine treatment for the patellar instability. Have done combined ACLR and MPFLR because dysplasia was mild. Have not yet had to do a trochleoplasty and an ACLR but would if dysplasia was severe enough.
@Patelladoc This is an 18 month post trochleoplasty and patella MACI knee MRI obtained to look at the MACI implant. Note the dramatic decrease in the TT-TG by only doing the trochleoplasty. This is chiefly because the trochleoplasty allows the tibia to derotate relative to the femur.
