My father Peter Yu died unattended in September 2025 in NORTH YORK GENERAL HOSPITAL. No overnight vitals for 12 hours. No telemetry for 72 hours. Unmonitored room despite his weakness. Implausible charting. We’ve hit nothing but roadblocks fighting for simple written answers — only closed-door meetings and vague letters. This isn’t just about my dad. It’s a safety warning for every family.

NORTH YORK GENERAL HOSPITAL
of Toronto Canada on Sep 28
due to **unhandled cardiac tamponade** for over 72 hours. No telemetry. No oxygen.

he died in an empty ward with no one attended.

The doctors and the hopstials have no answers and they’re still rightfully harming people everyday!!

No one told my mom he was that severe until we learnt from the medical records.

My mom collapsed on the road to the hospital in a morning when she heard my father passed.
