SIG M250 out of battery detonation. This is attributed to operator error, as during the clearing process, the gunner holding the charging handle to the rear was not far enough to lock on the sear. The handle slipped, a belt in the tray, and a live round stuck in the barrel.
M240B cracked steel receiver. This occurred during suppressor development. The firing schedule was well outside the TM, with a 600-round continuous burst exceeding the prescribed limits, which may have contributed to the hydraulic buffer failure.

Even the steel receiver Bravo can fail under enough abuse and neglect. The hydraulic buffer assembly, with its poor design, unscrewed back into the stock, as the plunger was flush with the backplate.

Hard to believe, but we sometimes receive new M4A1 barrels. These are old Colt M4s upgraded to the M4A1 standard in 2018. One of the upgrades includes a SOCOM 14.5 inch heavy barrel.

M240 feed tray cover with an M249 feed lever return spring. A maintainer down the line used it possibly in a pinch, and it worked, though it wasn't the right one, so it was swapped out for the correct spring.

FN M249 op rod assemblies, showing two attachment methods for the operating rod. Both versions are FN cage code marked, the older design is welded, whereas the newer design is threaded and pinned. I have only seen the welded versions fail, with fractures occurring at the welds.

Kalashnikov Concern AK-203 features a tighter bore, a slower twist rate, and a taper step in the chamber throat. The reported accuracy improved from roughly 3-5 MOA (AK-103) to 2-3 MOA, which is roughly a 33-40% increase in practical accuracy.
