Updates from the WHO Classification of Breast Tumors, 6th Edition (not published yet)

The WHO 5th edition recommended that encapsulated papillary carcinoma (EPC) with high nuclear grade, increased mitotic activity, and/or a triple-negative or HER2-positive phenotype be graded, staged, and managed as invasive disease

In the 6th edition, the WHO now recognizes the existence of rare forms of EPC with high-grade features or ER negativity that may still be categorized as EPC
Such tumors may be encountered on core biopsy; classifying them as invasive could lead to inadvertent neoadjuvant therapy

The vast majority of EPCs should be low- to intermediate-nuclear grade and ER-positive

3. E-cadherin isn’t enough

E-cadherin is one of the most useful immunostains when distinguishing invasive breast carcinoma NST from invasive lobular carcinoma—but it should never be interpreted in isolation.

A panel is often more informative:

- NST: membranous E-cadherin, β-catenin, and p120 (Case A 👇)
- Lobular carcinoma: loss of E-cadherin and β-catenin, cytoplasmic p120 (Case B 👇)

Be aware of aberrant staining patterns. As always, morphology comes first—immunohistochemistry should support, not replace, the diagnosis.

They are inhibin positive, maybe a better stain that CD68
They are S100 and SOX10 positive CK and ER negative

Pitfall ⚠️
- They can look compressed/elongated (pic 3) instead of epithelioid like cells
- They can look like muscle (pic 4)

MMR or MSI? One of the most common areas of confusion in oncology and pathology.

A simple way to remember it:

🧬 MMR = Machinery (IHC)
🔬 MSI = Mistakes (PCR/NGS)

✅ IHC checks whether the DNA mismatch repair proteins (MLH1, PMS2, MSH2, MSH6) are present.
✅ PCR/NGS measures the genomic consequence—microsatellite instability.

High-yield takeaway:
• dMMR ≈ MSI-H
• pMMR ≈ MSS
• MSI-L is uncommon and is generally managed similarly to MSS for most clinical decisions, including immunotherapy selection in colorectal cancer.
