The fact that the administration is looking for opportunities to cut costs is great. These changes were needed. But they are missing an important piece.
The expectation that giving insurance companies more leverage in certain negotiations will lead them to passing on any cost reductions to plan holders or anywhere among their hundreds or thousands of subsidiaries, is ridiculous.
That is not their culture.
Remember. These are the same companies , when they were on the other side, tiering and other similar rules were passed for their PBMs, they sued and found other ways to charge new fees.
The administration has to go much further and make sure these changes are not gamed.
Not just by the insurers. Also by the hospitals.
They need to remove all confidentiality clauses from ALL contracts signed between insurance companies and at least non profit hospitals, if not all providers.
And require all of these contracts to be posted right next to the pricing hospitals post on their website. With the fines moved to 1m dollars per day for non adherence. Make it so it’s not a business decision to not adhere
The ONLY way you are going to know the carriers and hospitals followed through and adhered by the rules, without gaming them , is by making them publish their contracts.
With pricing. Terms. Everything.
Taxpayers are subsidizing both sides. Insurance conglomerates get 70 pct of their revenue from taxpayers.
Non profits keep billions that would go to taxpayers. They literally have more in cash than Medicare has in cash reserves.
We all have every right to see those contracts.
This will increase competition and cut costs more than anything.

Political self interest to get elected, takes precedent over anything some rich person self interest.
Being rich gets you in the door because you might give them money. What you want from that politician is meaningless if it doesn’t align with their self interest.
A Trump endorsement in a Republican primary almost always goes further than any amount of money or other influence anyone else can provide.

@KlausMarre They certainly will take their money. Who got elected because they supported crypto ? And isn't it possible they actually support crypto.
Giving money and getting nothing of value is the more likely outcome for wealthy donors
Think it through. You are the provider. A big ins carrier offers catastrophic plans at say 10k.
Providers know that 40pct of people can't afford $400 emergency bills. I know you think that paying lower pemiums should mean they have more money. Maybe they even put it into an HSA. Maybe they shop for cash prices.
But, what percent of people are that disciplined ?
Which means, exactly like today, providers will have to loan patients money.
They become subprime lenders to patients. Since they know, most people can't pay their OOP, they will raise prices so they can get into the pockets of their insurance providers faster.
Just like they do today where ACA deductibles average 3700 and max OOP is about the same as your catastrophic levels.
And, Just like they have inflated chargemasters just to game insurance pricing
And we also know the carriers are not usually good actors. So the number of denials will skyrocket and the price to appeal those details will skyrocket as well.
And the number of services they lazer out and don't cover, because they are, by definition, more expensive, will skyrocket
Leaving patients in the same mess they are in today.
@forgipper Unless you are a huge conglomerate that owns a provider, TPA, and other subs that can charge the carrier more, hitting their MLR sooner, but keeping the up charge in their subsidiary.

It seems crazy because the stock market is a multiple of where it was 20 years ago. Or even 6 years ago.
If you had a public company and a ton of founders stock, your net worth skyrocketed.

@mcuban The public isn't demonizing. The reality is that more wealth and income as a percentage of the economy is going to the top over the last 40 years. It wasn't always like this. Housing, education, healthcare costs are less affordable.
How do you expect people to react to this?
What about all the entrepreneurs that risked everything and failed ?
Or the ones that are grinding it out, trying to build a business ? You know, the ones that create more than 60 pct of the new jobs in this country.
The more successful they are, the more you hate them?
Take their money so they can't invest in other founders? Or invest in causes they believe in ?
And the ones that fail and get to deduct 3k a year of their losses ? Or went for it and are broke and might never see enough income to offset those losses ? You don't care if they go broke ?
10 pct of the people in this country are entrepreneurs that have started a business.

There are plenty of discussions to be had about income inequality. About helping people who are struggling economically.
I've talked about a lot of them in my timeline.
But in reality, saying "Eat the rich" will end up helping no one except maybe politicians trying to raise money. And probably hurt those in need, as more money is spent playing politics then helping
Wealth tax. Unearned income tax. Show me the multi year behavioral analysis, as opposed to simple models, that show more jobs, higher wages and personal wealth will be created for those who need it. I'd love to read them
Ideology is not a strategy. Nor is it a plan that has lasting impact on people who need help

I'm not a believer in trickle down. It's a joke.
I am a believer in trickle up. Where we do all we can get to get appreciable assets and higher wages into the hands of people who have to live paycheck to paycheck. I've said before I think raising the federal minimum wage to $20 is smart. When I heard people who worked for a company I invested in (but didn't run) needed government assistance, I made sure they all got raises. It was embarrassing to me that we didn't pay enough. I've made, or helped make, at least a thousand millionaires. And I'll keep working to increase that number
And that's the point.
The best path to help more people is to work with the people that can help more people.
If you demonize business people , you just make it harder for them to help anyone.
I'm not saying there aren't greedy, blood sucking business people that will do anything for a dollar.
But they are a small minority. Most entrepreneurs realize they are blessed. They want to find ways to help. They think they can use the same skills that built their business to help people.
And they can. They can hire more. They can give raises. They can educate. And they do all these things to help.
I've sat with Democrats and Republicans, incumbents and candidates,and asked them why they aren't meeting with businesses that make and sell the products that have become unaffordable, and asking them what can be done to reduce prices.
I do this continuously regarding healthcare. It's easy to see where the friction and increased pricing is coming from. Why (the politicians ) aren't you meeting with them and asking them to change ? Offering to support those that do raise wages, lower prices , improve benefits. Politicians just won't do it. They prefer the donations.
I did shark tank for 15 years purely to show people they can start small, grow their businesses, and help not only themselves, but their employees and their communities as well. And thats exactly what has happened.
Demonizing those that have success is counterproductive to every benefit you want to help bring to people.
