3 ENFERMEDADES MATAN NIÑOS EN ECUADOR:

1. FIEBRE AMARILLA
•Fisiopatología: Virus se replica en macrófagos → pasa al hígado → necrosis hepatocelular → ictericia y sangrado.
•Fases:
1.Inicial: fiebre, dolor muscular, náusea.
2.Remisión: mejora transitoria.
3.Tóxica: ictericia, sangrado, falla multiorgánica.
•Prevención: Vacuna 17D.
•Tratamiento: Solo soporte: hidratación, manejo de coagulopatías, UCI si hay disfunción hepática o renal.

2. LEPTOSPIROSIS
•Fisiopatología: Bacteria entra por piel o mucosas → llega por sangre a hígado, riñón, pulmón → vasculitis e inflamación sistémica.
•Fases:
1.Leptospirémica: fiebre, vómito, dolor muscular.
2.Inmunitaria: ictericia, hemorragias, falla renal.
•Prevención: Evitar aguas contaminadas, controlar roedores.
•Tratamiento:
•Leve: doxiciclina o amoxicilina.
•Grave: penicilina G IV o ceftriaxona.

3. TOSFERINA (PERTUSSIS)
•Fisiopatología: Bordetella pertussis produce toxina → paraliza cilios respiratorios → daño epitelial y secreciones densas.
•Fases:
1.Catarral: rinorrea, fiebre leve.
2.Paroxística: tos intensa, cianosis.
3.Convalecencia: recuperación lenta.
•Prevención: Vacuna DTP/Tdap, refuerzo en embarazadas.
• Tratamiento: Azitromicina (1ra línea), claritromicina o TMP-SMX si hay contraindicación.
