I've said it before and I'll say it again:
If you're sending a patient to the ED, please don't tell them you're sending them for admission, CT scan, etc. You are sending them for evaluation. What that entails is up to me. Inappropriate expectations leads to much discontent.

@JimKennett3 Suboptimal dosing of the acetaminophen component in these medication making it harder for patients to optimize the dosing of the acetaminophen. If i prescribe individual agents, I can have the pt take exactly the dose I want them to take of each agent.

1.8 million Ont residents do not have a family doctor. I don’t think increasing the residency length by 50 percent will help to close that gap nor incentivize perspective applicants. No one seems to want to talk publicly about this but everyone is talking. @allan_k_grillMD

@First10EM I agree with this 100 percent. I try to teach every trainee I have about how and when to apply the rule. Feeding every bump to the head through the rule is a bad approach that I see too often.

@First10EM I’m not convinced of the results of this study, and I don’t think the authors are either. I actually think it would be challenging to increase imaging utilization if the rule is applied correctly. If applied incorrectly, imaging utilization likely goes through the roof.

@First10EM Also, the Stiell et al. implementation study provides enough discussion and explanation of why the results may be as they are to make me look to other studies. I don’t think this study in isolation supports abandonment of the rule.

@First10EM This is one study (admittedly a high quality one). PMID 28739290 shows 5 percent reduction and PMID 25572644 shows 8 percent reduction. Just 2 examples. Yes the evidence level is lower but this doesn’t mean they should be discounted.
