LAPA 3142 - One of the most controversial air crashes in Latin American History

August 31 1999: LAPA Flight 3142, a B-737, crashes in Buenos Aires (Argentina). 63 of 100 aboard, 2 on the ground die. On take-off run, the jetliner failed to rotate, left the runway, hit buildings, and caught fire.

Inquiry found that pilots breached the pre-flight SOP – including with informal conversations – and as such, forgot to extend the flaps for takeoff. Even though the Takeoff Warning System worked and triggered an alarm about the failure, the crew dismissed it instead of aborting the maneuver.

The tragedy triggered major criminal proceedings in Argentina. In 2000, several senior LAPA executives and military officials were indicted for criminal negligence.

Although the trials did not produce convictions, the disaster brought reforms, including the demise of LAPA airlines and the transfer of oversight of Argentine commercial aviation from the military to the civilian sector.

The disaster was later dramatized in the acclaimed 2004 film *Whisky Romeo Zulu*, directed by a former LAPA pilot, who had resigned shortly before the accident after repeatedly warning management about serious safety violations
