Agree with limiting radiation and avoiding unnecessary scans. Also agree that observation and non-ionizing studies can be alternatives. But a lot of this literature about radiation is being misunderstood.

Receiving 20 head CTs *is* clearly a risk factor for later cancer; but how much is whatever underlying condition led to getting the scans? This is a significant residual confounder.

The study reporting a 1 in 10,000 risk of hematogenous malignancy after one head CT? Harder to interpret what this means. A lot of the studies use the linear no threshold hypothesis for radiation exposure, which is probably not true biologically. There’s significant debate about what the risk truly is with single lower dose exposures, like a single CT scan.

For context, lots of things involve radiation expose. Eating bananas. Living in a house made of stone. One head CT is about the same radiation dose as 100 hours of commercial air travel. I wouldn’t leave my kids home on vacation or take away their bananas to mitigate those risks though.

My concern with posts like this is there’s a tendency to overestimate the risks of scary radiation and underestimate the risks of missed diagnosis. Clinicians are aware of both risks when ordering. Clinical decision rules exist to help decide if a head CT is indicated.
