it shouldn't be surprising that management doesn't get Agile. It'd be more surprising if they did. The most popular method (Scrum) focuses on the team, ignores management & doesn't really recognize lean. The most popular large scale method is development focused not biz focused.
I think TOC has an important message about DevOps. We can optimize deploying web pages and services to hell and back, but that's not the constraint for most teams, it never was. If the industry never addresses the database upgrade problem things will never get better.
