Dr. Mahnoor Fatima and Dr. Naseema Gangat: One of the largest real-world analyses to date examined 356 newly diagnosed AML patients who failed frontline HMA+venetoclax across the Mayo Clinic enterprise.
1. Outcomes after HMA+VEN failure remain poor, with median survival measured in months ( 4 mo) highlighting a major unmet need in AML.
2. Independent adverse prognostic factors included peripheral blasts ≥20% and complex/monosomal karyotype, while wild-type IDH1 and allogeneic transplant after failure were associated with improved survival.
3. Among salvage approaches, no specific regimen demonstrated a clear survival advantage, emphasizing the need for novel therapeutic strategies and clinical trial enrollment after HMA+VEN failure.
