@Lpa_Doc What is most important, to have a lower ApoB and Lp(a) or a lower Lp(a)/ApoB ratio? I would think a lower absolute number is better. My ApoB is less than half (24 vs 58) on combo Repatha Crestor 20 than Repatha alone. My Lp(a) is actually slightly lower on the combo (45vs 48).

@Drlipid How to balance lowering LDLc vs Lp(a)? 2yrs p nSTEMI on Crestor 40 qd. LDL from 70 to 38, but Lp(a) 48 to 83. I recently added psk9i LDL went to 11 and Lp(a) to 50. If I drop Crestor to 20 or 10, LDL may go up, but Lp(a) should come down. Thoughts?

@Drlipid Got it. The cholesterol mass is only a small part of Lp(a) mass. So presumably a small part of LDLc mass.

@mohelwan2022 @Drlipid Statins can cause an elevation of Lp(a). Normally it is only 10-20%. Mine is 70%. I don’t know if a mechanism is known. I started psk9i. I’ll see what that does.

@MichaelJBlaha How about 60s male, LDLc 85-90, pos family history on no treatment has a nSTEMI? 90% LAD, 100% RCA. CABGX3. Lp(a) not measured until post op, 48 mg/dl. LDLc on max statin 38 mg/dl.

@EricTopol @youyanggu I guess it is keep the curve flat to prevent overwhelming the healthcare “system” while we work to get treatments to decrease morbidity and mortality and a vaccine. Meanwhile we let the economy return somewhat and accept 100,000 deaths every 4 months until treatments, vaccine

One surprising result is the early flattening of the dislocation curve with DA and continued dislocation risk beyond 3 months and more with PL.
