While working on a new paper, I've been re-reading old papers on #POTS: one is from Dr. Low et al of Mayo Clinic who was one of the first to describe it. Years later we have crazy debates on what POTS is, with one senior researcher recently saying in a meeting that "it's a controversial diagnosis" while some other physicians attempt to lump it with functional neurologic disorders. This paragraph should be re-read by all of us in this field to remind ourselves what POTS really is: a restricted form of autonomic #neuropathy in at least 50% of patients.

Cardiovascular outcomes in long COVID-19: a systematic review and meta-analysis
🔥Meta-analysis including nearly 10 million patients, only confirming what we already know:
➡️Long COVID-19 was associated with a higher risk of
- thromboembolic disorders [HR 3.12 (1.60, 6.08)],
- coronary heart disease [HR 1.61 (1.13, 2.31)],
- stroke [HR 1.71 (1.07,2.72)],
- arrhythmia [HR 1.60 (1.13, 2.26)],
- cardiomyopathy [HR 1.71 (1.12, 2.61)],
- myocarditis [HR 6.11 (4.17,8.94)],
- hypertension [HR 1.70 (1.56, 1.85)],
- heart failure [HR 1.72 (1.15,2.59)]
- and cardiogenic shock [HR 2.09 (1.53,2.86)] compared to non-COVID-19 controls.
➡️The risk of cardiovascular disease increased significantly four weeks or more after recovering from acute COVID-19
➡️This meta-analysis showed that the cardiovascular risk burden of long-term COVID-19 is significant and spans multiple categories of cardiovascular disease (ischemic and non-ischemic heart disease, arrhythmias, etc.).
➡️Care for survivors of COVID-19 after acute attack should include attention to cardiovascular health and disease.
🤔Caution, study has some well noted limitations and only covers ONE C19 infection!

➡️Moderate to severe autonomic dysfunction in 75%
➡️Abnormal cerebral blood flow (CBF) was seen in 61%
➡️There is a high prevalence of reduced cerebral perfusion in those with POTS, which is detectable with supine brain SPECT imaging and is associated with diminished quality of life.
➡️The most frequently affected areas are prefrontal lateral and sensorimotor cortices.🤔🤔🤔
➡️Reduced cerebral perfusion appears to be more prevalent in those with comorbid joint hypermobility syndrome and independent of POTS triggers.
➡️These results highlight the need for further research to delineate the etiology and mechanistic pathways underlying neurovascular dysfunction in post-viral and autonomic syndromes including long-COVID.
