If you do not follow those punishments the severity climbs until you are physically made to take punishment. The Church set the laws and enforced them in much the same way we do now.

Being declared a heretic was a death sentence, if you were a ruler excommunication meant you would soon be overthrown & killed. Not to mention crusades, inquisition, counter reformation, etc
