Teaching point for y’all out there. Eye drops can go in ears but ear drops can’t go in eyes. My pediatrician prescribed ear drops (ciprodex) for my kid,$180. Cipro eye drops: $12. Think about this when prescribing for your patients. Eye drops are often cheaper
Today in health insurance, @Cigna declined to cover my daughter’s urine culture done when she was 3.5 years old. Said she was not “non-verbal” & otherwise healthy. @Cigna you ever talk to a 3 year old about medical issues? Done a full history? Sounds like a @DGlaucomflecken skit

Some people talk about how, as a physician, I should not inject politics into medicine, but then you see stuff like this and clearly someone is doing it anyway
Or if the lady had a dead/dying fetus and telling her she had to keep carrying it. These people don’t give a two foleys about hurting other people in their absolutism.
A big mental trauma I experienced as an ER resident was a woman miscarrying at 17 wk (my wife was 24 wk at the time). patient was traumatized. I had to collect the fetus in a “specimen” bottle bc it was <20 wk. I can’t imagine forcing someone to carry a no viable fetus to term.

I then had to figure out if I had to do a death certificate (no). She asked to hold the baby. Then I had to awkwardly take it and put it in the same type of cup we used for urine collections (specimen cups). I can’t imagine if I had to worry about being prosecuted
My suspicion is that a major source of burnout in medicine is when systems are in place that devalues your professional opinion and makes you feel helpless and unable to do the right thing for patients

While I get that residents need to work hours and shifts and see patients, we don’t need to do it in a malicious and exploitive manner. Unsafe for residents. Unsafe for patients.
