His pants (not underwear) got caught and fell, making them recreate the same accidental kiss gag. Sasuke pukes, Naruto gets angry, and Ino gets confused (that's the joke).

@Aznkewl@benxard Naruto is not you, he is a fictional character. They were also classmates for 6 years and bonded as orphans for 5 years before Team 7. And they started feeling like real siblings years before Team 7.

@theedorksinlove By the way, this is the bond he is talking about. Sasuke was moved by Naruto calling him his brother. He wore his headband (a callback to the hospital fight) as a way to acknowledge Naruto, but he still wanted the Mangekyo, so he said he would break it (kill him) to obtain it.
