During the last FY our Integrated Healthcare team provided 6,702 occasions of direct clinical nursing care to 954 people. 1/3
The integration of nurses into our Teams is informed by a housing first approach that recognises a broad range of social factors impact upon people’s health. To achieve outcomes for vulnerable people exp. homelessness, housing and social support services are delivered. 2/3
