Most hacks I have seen attack the centralization links in the chain - they are the weakest ones arguably.

1. The prevalent single-source oracles are constantly exploited by flash loan attacks

2. Social engineering has been targeting centralizations like admin keys for smart contract upgrades, low threshold multi sigs, etc

3. Centralized keepers not showing up -> debts piling up and the spiral down of a bank run
