The age old "rule out pneumonia" ie rule out non-viral pneumonia.... particularly in toddlers this time of year-do we actually have legitimate studies proving how accurate a one view or a two view chest x-ray actually is in correlation to this specific question. I'm afraid radiographs are not particularly accurate as many believe. Does lack of lobar or sub lobar so-called consolidation on x-ray actually, truly, with any degree of certainty, exclude a bacterial pneumonia? I know I know, we're talking bread and butter stuff here but still! I want to see the studies.

When your senior resident or attending tells you to order an imaging test, make sure you understand the purpose. If you don't, ask them! What are you hoping to answer with the study?
A compliant radiologist follows the Fleischner Society Guidelines religiously. A valuable radiologist ignores them when necessary. If you measure a radiologist’s quality by adherence to guidelines you punish the radiologist who thinks contextually
2nd year radiology residents - your sensitivity should be 98 % & specificity 75 %
4th year radiology residents - your sensitivity should be 94 % & specificity 90 %
