✂️Appendicectomy for Maintenance of Remission in Ulcerative Colitis: The ACCURE Trial
🌸Previous observational studies suggested a protective effect of appendicectomy on UC.
🌸 ACCURE Trial: First randomized controlled trial to evaluate appendicectomy as an adjunctive therapy.
Type: Pragmatic, open-label, multicenter, randomized controlled trial.
Countries: Netherlands, Ireland, UK.
Patients: 197 with UC in clinical remission but recent relapse.
Intervention Group: Appendicectomy + standard therapy.
Control Group: Standard therapy alone.
Outcome Measured: Proportion of patients with disease relapse at 1 year.
🌸 Relapse Rate:
Appendicectomy Group: 36%
Control Group: 56%
Relative Risk: 0.65 (95% CI 0.47–0.89
p-value: 0.005
🌸 Strengths
Randomized controlled trial design.
Multinational, real-world setting.
Statistically significant reduction in relapse rate.

🧨Open-label design (risk of bias-performance bias,participation bias and detection bias).
🧨No sham surgery control.
🧨Short follow-up period (1 year).
🧨 Mechanism of action not fully explored.
🌸 Clinical Implications
Appendicectomy may be a viable adjunct in patients with frequent UC relapses.
Should be considered case-by-case, balancing benefits vs surgical risks.
Not yet a standard of care.
