—> Dr Cheah, a non-anaesthesiologist will decide if a mode of Anaesthesia suitable or not for a patient (LA vs GA) and Anaesthesiologist would have to request and abide by whatever decision by TPA, or else it wont be paid by insurance

Now, usually, when GL is requested by the surgeon, for whatever surgery going to be done in Daycare, the insurance will look into the request and approve the GL for the surgeon to do whatever needed for the patient.

Then when the day comes, Anaesthesiologist will decide the mode of Anaesthesia suitable for the patient. Now, insurer want types of anaesthesia to be approved in addition to types of surgery.

Mind you, the type of surgery is not the only indication for LA or GA. Other patient factors need to take into account also, which may makes the patient not suitable for LA even if the surgery seem “simple” and “daycare”.

I could give plenty of example and scenario of this.

In addition, even if you “approve” beforehand the mode of Anaesthesia, when the surgery day comes, plenty of other factors necessitate the LA method to be changed to GA, on the day of surgery. So now what?

Do we cancel the surgery? Do we postpone and ask for a new approval?

Where do we draw the line when insurance dictate every decision by the specialists?
