(Then) the worst air crash in Brazil
June 8 1982: VASP Flight 168, a Boeing 727, crashes in Fortaleza (Brazil). All 137 aboard die. The jetliner impacted a mountain on approach at night. Enquiry stated Captain became fixated on the city lights, descended below Minimum Sector Altitude, ignored two terrain alerts, and co-pilot's warning of mountains ahead (more details below)
The video seen here is from a reenactment by TV show GC Mais. Note it uses the original CVR recordings, which may be offensive to some viewers*. Also note that the producers had to “brighten” the background so the aircraft could be visible, the accident occurred at night, and the obstacles were not as visible.

“Flight 168 completed its first leg from São Paulo to Rio de Janeiro without incident. The aircraft then departed Rio de Janeiro for Fortaleza. As it approached the destination, the crew was cleared to descend from cruising altitude of flight level 330 (approximately 33,000 feet or 10,000 meters) to 5,000 feet (1,500 meters).
Operating at night with the lights of Fortaleza visible ahead, the Boeing 727 descended through its 5,000-foot clearance limit and continued descending until it struck a mountainside at 2,500 feet (760 meters), killing all 137 people on board.
The investigation found that the captain, possibly disoriented by the bright lights of the city ahead, continued the descent well below the authorized 5,000-foot limit despite two warnings from the ground proximity warning system and the co-pilot about the rising terrain. The aircraft struck a wooded mountainside at 2,500 feet (760 meters) and crashed”
*As a note, googling for this OTD, found a lot of graphic footage of the crash site. Journalism was different those days, I guess.
