Why do marathon runners get injured?!
Findings from a recent study of over 700 runners

Something I don't understand about anti-vaxxers in America:
They reject vaccines and presumably modern medicine, but rather than lobby for better healthcare research or reformation of the healthcare system - they're okay with the government withdrawing huge funds from healthcare?
Six youths have filed a judicial review against Putrajaya, calling for urgent measures to stop deforestation and ensure at least 50% forest cover.
The group is seeking a mandamus order requiring the government to act, with progress updates every 90 days, and a declaration that their constitutional rights to life and equality have been violated.

Progressing rehab can be challenging if symptoms remain irritable. Here's an approach that's often effective and builds from what's manageable towards individual goals.
The heavy isometric programme for Achilles Tendinopathy from Radovanonić et al. (2022) has somewhat gone under the radar but has lots of potential benefits 💪
It’s just one exercise and takes less than 25 mins per week!
Chronic musculoskeletal (MSK) pain affects 1.7 billion people globally and costs hundreds of billions annually.
Exercise helps ✅
⚠️but: evidence base is too imprecise to implement it well

Lovely thread
Radicular pain is fascinating - & unpredictable
20 years experience of this clinically - & then (unfortunately) my own experience has reinforced this
Me - textbook L5 distribution pain; i could almost trace the pain path with a pen - following the sciatic nerve, from posterolateral buttock, around the fibula head to the foot.
A pt the other day - far lateral L4 compression (these can be hugely painful adjacent to the DRG) - disabling lancinating pain, only to groin & thigh 🤷♂️
Understandably the focus being on the hip joint

This foot was asymptomatic, but showed signs of FHL tightness/weakness. These tests are a slightly crude measure of strength/function, but can be useful as a cluster to help understand the tissues in & around the big toe when teamed with a thorough subjective history.
Introducing a Novel Diagnostic Tool: The "broken wing sign" is a newly developed, non-weight-bearing physical examination test to detect gluteus medius tendon tears and muscle degeneration.
High Diagnostic Accuracy: The sign demonstrated high accuracy for detecting any gluteus medius tear, with an overall sensitivity of 81.8%, specificity of 80.0%, and a positive predictive value (PPV) of 91.8%.
Detects Severe Pathology: The test is particularly useful for identifying severe gluteus medius issues. An external rotation of ≥30∘
(a "highly positive" sign) yielded 100% specificity and 100% PPV for a tear, and this degree of rotation strongly suggests a severe, likely massive tear requiring complex reconstruction.

Not every training stress is created equal. Some are very neurally demanding, some lean more metabolic, some are just lower intensity by design.
This chart is simply a framework for how I tend to view and organize them. It’s not 100% universal…acceleration, for example,
