The tip volume was too full and spread out, which widened the whole nose
and showed the nostrils from the front. Reducing the tip alone would
have left the base wide, so both were addressed together.

What was corrected:
· Nasal width and nostril show reduced
· Upturned tip improved, angle corrected for a shorter-looking philtrum
· A natural dorsal line created

The patient asked for a result that would not look done. That framed
every decision here — the aim was a natural finish rather than the
largest possible change.

Results vary from person to person, and what suits one face does not
suit another.

Why stem cell therapy comes first.

Acute inflammation and contracture had hardened the skin tissue. In that
state an operation is not a question of technique — the skin simply
cannot take a reconstruction. So the nose was not operated on first.

Stem cell therapy came first, and four months later the skin had
softened to an operable state. Only then was the reconstruction done.

What the reconstruction involved:
· Thickened scar tissue removed
· Septum shifted left to balance proportions
· Columella lowered, angle corrected
· Autologous rib reconstruction

The comparison shown is after stem cell therapy and immediately after
surgery. Swelling at this stage is expected.

Whether a nose needs this staged approach depends on the state of the
skin, and that is different for every patient.

Three things were working against each other here. The nose read
upturned, the dorsal line ran irregular, and the columella was both
thick and wide — which is why narrowing it alone would not have fixed
the front view.

What was corrected:

· Nasolabial angle
· Nostril symmetry

Revision cases are planned around what previous surgery left behind.
How much can change depends on the remaining structure, and that is
different for every patient.
