Why is the 4th toe the most commonly affected digit in psoriatic dactylitis?
The exact reason remains unclear, but current evidence points towards a combination of:
• Repetitive biomechanical stress (“deep Koebner phenomenon”)
• Enthesis-rich anatomy of the 4th toe
• Flexor tenosynovitis as the dominant lesion
• Diffuse soft tissue inflammation, rather than isolated synovitis

Clinical Pearl:
A painful, swollen 4th toe (“sausage digit”) should always prompt evaluation for psoriatic arthritis, even in the absence of obvious skin psoriasis.
