We give O₂ to almost every AHF patient. Guidelines say start if SpO₂ <90%. But in practice, most patients are targeted to ≥95% or higher — without RCT evidence.
📋 Design:
Phase II/III | RCT | Triple-blind | Parallel-group
Sponsor: Copenhagen University Hospital, Hvidovre, Denmark
👥 Population: Adults admitted to ED with AHF + pulmonary congestion + hypoxaemia
n = 122
🔀 Arms (1:1):
▪ Liberal: SpO₂ target 96%
▪ Restrictive: SpO₂ target 90%
Key innovation: Blinding achieved via O2MATIC — a closed-loop automated oxygen delivery robot
→ Titrates O₂ every second to the prespecified target
→ Eliminates manual titration bias
→ Maintains SpO₂ within target ~87% of the time vs ~61% with manual care
Why it matters:
Hyperoxia → ↑ coronary vascular resistance, ↑ SVR, ↓ cardiac output
Normoxic AHF patients given O₂ → no survival benefit, longer LOS
But no powered RCT has ever tested this in AHF. Until now.
