One of the joys with giving International lectures is learning what questions the audience is curious about. It helps me appreciate how advanced our technologies are around the world, and provides an opportunity to be stumped to inspire a new learning opportunity. Both happened today! 🇸🇦

WCLS 2023: IMRT Reduces Pulmonary Toxicity, Radiation Exposure to the Heart Versus 3D-CRT in Patients With NSCLC | PracticeUpdate https://t.co/zM9cA8HRpX
Amazing talk by Maria van Rijn-Dekker at #ESTRO2023 on Adaption of treatment planning to reduce xerostomia in patients with head and neck carcinoma- such an important topic for quality of life! thanks especially for the 'how-to' Information
“ RCT is not reality. It is a modelling of the reality. … selection is the Key! …guideline is a surrogate of evidence… only as good as the evidence reviewed. 👍presidential symposium-“ the Evidence gap” #ESTRO2023
Brilliant lecture from @SorenBentzen during the Presidential symposium at #ESTRO2023, highlighting the need for randomized evidence in radiotherapy. 'Guidelines are not evidence-based, they are a surrogate for evidence'
Masterful presidential session talk (as always!) by the amazing @SorenBentzen about the need for high quality evidence-based medicine and a word of caution that randomized trial data should be critically assessed and not always taken at face value #ESTRO2023
Hans Langendijk at #ESTRO2023 on Comprehensive Toxicity Profile:
- Personalized approach to optimize dose distribution in HNC pts
- Reduction of severe acute and late toxicities
- Potential estimate of the benefits of PBT
- Oral cavity as the most important OAR in HN RT
Fantastic talk by @SahgalArjun on potential for patient benefit from MRI-guided adaptive RT in GBM. Could methylation status predict who benefits most? #ESTRO2023
Do we need to reduce PTV to spare OARs? 2nd analysis of COMET 5 ph 2 trial.
36% PTVs under covered.
Under coverage did not predict worse LR control of PFS.
GTV likely still received full dose & systemic therapy may contribute to this.
So yes- safety first !#ESTRO2023
#ESTRO2023- 50 mo follow up from DESTROY fractionation trial from Carole Mercier. Basket histo study of lymph and bone targets. mPFS lower for 5 fx SABR group (?But mostly distant failure, so unclear attribution). Nice to see the conclusion that single fraction 🥊is preferred!
It’s always brilliant to see the STAR-TREC phase II data - here expertly presented by Prof Marijnen. Phase III is recruiting very well - with >210 pts randomised in 3 countries to the organ preservation arms #ESTRO2023#rectalcancer#organpreservation
Dr Stephanie Kroeze presents a Delphi consensus of combination systemic therapy and SRS/SABR. ➡️Consensus to interrupt with targeted therapy
➡️large body of evidence with checkpoint blockade (ICI)
➡️ but why interrupt ICI for a week? Half-life of many, many weeks… 🧐 #ESTRO2023
@DrAndrewLoblaw presents a fantastic Overview of dose escalation in prostate cancer and highlights the extremely important ASCENDE SBRT study which will open later this year.
