The I Do Not Consent Form™ doesn't get "lost" or "grow legs" because it is delivered to the hospital in a very legal way to ensure it get's uploaded into the hospital electronic medical record, per the accompany cover letter to the hospital CEO. When delivered as directed, the patient has 2 proofs of receipt. They know you KNOW that they received it and if challenged have "evidence."
