❌ “Bleeding a lot”

WHY AVOID IT:

“Bleeding a lot” is subjective. One nurse’s interpretation of “a lot” may differ from another’s. It doesn’t describe the severity or provide measurable information.

✅ CHART INSTEAD:

@ 1700 Dressing saturated with approximately 75mL of bright red drainage. Active oozing noted from the surgical incision.

Sterile 4×4 gauze applied and secured with a pressure dressing.

Vital signs obtained: BP 104/60 mmHg, HR 102 bpm, RR 20 breaths/min, Temp 36.8°C, SpO₂ 99% on 2Lits of Oxygen,Patient alert and oriented ×4.

Skin warm and dry,Provider notified.

Will continue to monitor dressing, drainage, and hemodynamic status.

I wish people understand that blood relatives can still mean nothing in the grand scheme of things.
They could be your enemies and they could actually not even be in your life.
This is why I always suggest video wills.
Make a video of exactly what you expect after your death so that people don’t confuse your enemies for your family because of relation by blood.
