The crus cerebri is not synonymous with the cerebral peduncle, and the corticospinal tract is not synonymous with the crus cerebri.

Traditionally, the cerebral peduncle referred to all structures anterior to the midbrain tectum:
the midbrain tegmentum, substantia nigra, and the crus cerebri.
The crus cerebri itself is simply the most anterior portion, sitting directly in front of the substantia nigra.

What travels through the crus cerebri?
A dense highway of descending fibers from the neocortex to the brainstem, cerebellum, and spinal cord—
but its internal layout allows remarkably precise localization.
Using my labeled diagram (A → D) from lateral to medial:

A: Temporopontine fibers (most lateral)

Carry temporal lobe input to the pons → synapse → decussate → ascend to the contralateral cerebellum.
Damage → contralateral limb ataxia, dysmetria, or clumsiness.
B: Lateral corticospinal fibers

Major descending motor pathway for limb movement.
Damage → contralateral arm/leg weakness.

C: Corticobulbar fibers

Control bulbar muscles for swallowing, speech, and airway protection.
Damage → severe dysarthria, aspiration, and inability to manage secretions.
D: Frontopontine fibers (most medial)

Originate in the frontal lobe → synapse in the pons → decussate → reach the contralateral cerebellum.
Damage → contralateral limb ataxia.

The temporopontine, frontopontine, parietopontine, and occipitopontine fibers form the cerebro-pontine system (aka the corticopontocerebellar tract), all crossing to the contralateral cerebellum to fine-tune movement.
This is why crus cerebri lesions may produce ataxic hemiparesis, depending on which layer is affected.
Back to the patient.

The infarct involves the lateral portion of the right crus cerebri — corresponding to regions A + B.

His exam fits perfectly:
Left dysdiadochokinesia / limb clumsiness → involvement of lateral cortico-pontine pathways to the left cerebellum

4/5 left grip → partial involvement of lateral corticospinal fibers

A clean localization:
Right lateral crus cerebri infarct → contralateral limb clumsiness + mild weakness.

🚨 What about the rest of the midbrain tegmentum?
Earlier I discussed the tectum and its role in upward gaze — and today we covered the crus cerebri, the most anterior layer of the cerebral peduncle.

But the midbrain has more to reveal:

Substantia nigra

Red nucleus

Oculomotor nucleus

Edinger–Westphal nucleus

And yes — there is a major functional difference between the Edinger–Westphal nucleus and the Oculomotor nucleus.
