Response to @RCPhysicians ask re @lengreview clinical protocol done- as a Fellow of RCP.
I believe in transparency- so here are my views on some of the key questions.
In summary?
Needs a rejection on key areas of PAs seeing undifferentiated patients.
And certainly not lean on Gen Next to be liability sponge.
And let @rcgp make their own decisions regards primary care- no need to interfere.
NB:
Don't like it, give a counter view.
Don't abuse or be snarky. It's a democracy- it's my view. You are welcome to give yours.

As a doctor…I knew
As a human…I hoped different.
Desperately.
But I knew.
Amidst all the beeps in that ITU.
I knew.
The scans told the medically trained brain enough.
And since that day?
I stopped believing in many things
Miracles being one of them.
SGLT2i became part of “standard” #T2Diabetes care when the class of medication became generic
GLP-1 use as “standard” #obesity care will happen when it goes generic
The money for any else regards strategy?
Isn’t there.
Everyone knows it
But won’t say it openly.
Fact.
@Gerogrios All I can say is that I don't feel that.
I can't account for others.
I know my limitations- and happy with it too.
Good you are worried about the lack of SOP- pick that up with those who should do that in that case- rather than come to my tweet?
I will say this- again.
In a safety critical 'industry' like healthcare?
Lesser trained staff usage-at scale-will cause harm.
You will 'get away with it' a lot of times.
Yet all it needs is 'one miss'.
And the higher training reduces the chance of that 'one' - it's what training is all about.
If you don't understand that?
You fail safety issues
And you fail as an educator and trainer.
Thank you @JanetEastham for continuing to raise the issue.
The irony?
Every single person responsible for such decisions regards 'what the public can have' - reaches out to a doctor when they need help for their loved one.
'Cake for me. None for you.'
Dylan Cope died after an A&E nurse practitioner mistook his abdominal pain for flu, having ignored his GP’s referral for suspected appendicitis. His family thought she was a doctor.
A coroner found he would have survived had a senior doctor examined him.

@Gerogrios anyone working outside their competence based on their training- is a quack.
I would be one if I tried doing a session as a physiotherapist.
Or a GP.
Or a Podiatrist.
Hope that helps.
Be more worried about safety & death than my tweets.

Trivia yet of utmost significance
A saviour of millions of lives #gbdoc
"It seems to me that a decision has been made to replace doctors and to remove distinctions between doctors and other professions.
It's no coincidence that the NHS continues to struggle with efficiency, productivity, and safety problems while we progress further towards a blended workforce without the necessary training supervision or regulation."

Just do things that are…
Kind
And makes you..emotional
And you know humanity isn’t lost.
However bleak it sometimes appears to be
