In some cases removal of ICD is impractical and is associated with significant risk. In these cases close observation and surveillance blood cultures might reflect balance of risks.

@syctong I had similar case of multi-relapsing pan-sens pseudo in BJI. 6ft, 120kg human. TDM - piptaz cleared at 6 hours. Cure with 18g continuous infusion - 6 weeks. 12 hour urine creatinine - clearance was >190/mLmin (no obvious physiological state for augmented clearance etc).
