Subject: Request for Immediate Intervention Regarding Patient Care, Family Access, and Alleged Reprisal Against Family Advocacy
Dear Dr. Frank Martino,
I am writing as a last resort to seek your urgent intervention regarding the care and treatment of our 83-year-old mother, who suffered an ischemic stroke on January 19, 2026. Following treatment at Sunnybrook Hospital, she was repatriated to Etobicoke General Hospital on January 23, 2026.
As a result of her stroke, our mother has been left paralyzed with no mobility. Despite her physical limitations, she remains cognitively aware and able to communicate her needs and concerns. I am her primary caregiver, and my sister, who herself has a disability and uses a wheelchair, assists whenever possible.
Since our mother's admission, our family has witnessed numerous incidents that have caused serious concerns regarding the quality of care, patient safety, and overall treatment she has received. We have repeatedly advocated for her wellbeing and safety. Rather than addressing these concerns constructively, it appears that our advocacy has resulted in retaliatory actions against our family.
For nearly a month, I have been prohibited from visiting my mother through the issuance of a trespass notice. This separation has caused significant emotional distress to both my mother and our family. My mother has repeatedly expressed through phone calls that she misses me deeply, cries frequently, fears she may never see me again, and worries about how she will ever be safely discharged without my involvement in her care planning. She has stated that she fears she may pass away without seeing me again.
I wish to emphasize that I have never threatened, assaulted, or acted physically aggressively toward any staff member. Police were called regarding this matter and declined to take any action against me. My understanding is that officers did not find grounds for criminal charges and questioned the necessity of a trespass order under the circumstances.
The Patient Ombudsman and government representatives have also been contacted regarding this matter. The concerns raised are serious and extend beyond a simple disagreement between family members and hospital staff. The current restrictions effectively prevent me from supporting my mother during one of the most vulnerable periods of her life.
Our concerns regarding care are further compounded by an unexplained injury that occurred in March 2026. Our mother developed a significant wound to her Achilles tendon, which has negatively impacted her recovery and rehabilitation progress. She remains under the care of the hospital's vascular surgery team and nurse practitioner.
To date, our family has not received a satisfactory explanation regarding how this injury occurred. Our mother reported that she was placed in a wheelchair without appropriate foot protection and informed a physiotherapist that her foot was hurting. According to her account, these concerns were not adequately addressed. Subsequently, she developed a serious wound requiring ongoing medical attention.
Our mother was previously an independent, vibrant, and active individual. The combination of her stroke, the subsequent injury, and the emotional distress caused by separation from her family has significantly affected her quality of life and recovery.
We are also deeply concerned that our family has been treated differently because we are Black. We have therefore sought legal advice and have filed concerns with the Ontario Human Rights Commission. We believe these matters warrant careful review to determine whether discrimination, bias, or differential treatment may have contributed to the actions taken against our family.
We respectfully request your immediate intervention and review of the following:
1 The immediate reconsideration and removal of the trespass order preventing me from visiting my mother.
