ECG From Scratch #15: What an ECG Can’t Tell You
The ECG is powerful, but it has limits.
Before we move to ECG leads and axis, remember these 5 principles:
1. ECG ≠ mechanical function
An ECG records electrical activity, not how well the heart is pumping.
A patient with severe pulmonary edema can have a relatively normal ECG.
Conversely, an abnormal ECG does not automatically mean poor cardiac function.
2. ECG ≠ direct structural imaging
The ECG does not directly show valves, septal defects, or chamber anatomy.
Instead, structural disease can produce characteristic electrical changes that allow certain diagnoses to be suspected, such as mitral stenosis, pulmonary embolism, or myocardial ischemia/infarction.
3. The ECG doesn't record everything
The clinical ECG is a surface recording of the summed electrical activity reaching the electrodes.
Some electrical activity is too small to appear clearly, while opposing electrical forces can partially cancel each other.
That is why significant myocardial ischemia can sometimes occur despite a normal or nonspecific 12-lead ECG.
4. Some electrical activity needs special recording
The SA node and AV node are not directly visible on the standard surface ECG.
Electrical activity around the AV junction can be recorded with specialized intracardiac techniques, such as a His-bundle electrogram.
5. A normal ECG doesn't rule out disease
Some important abnormalities can be intermittent.
Eg include:
⭕ Intermittent high-grade/complete AV block
⭕ Paroxysmal or sustained ventricular tachycardia
⭕ Transient myocardial ischemia
So a single normal ECG is only a snapshot in time.
The 2026 ESC Heart Failure Guidelines just changed how we classify and treat heart failure.
One major shift: HFmrEF is gone.
HFrEF now extends up to LVEF 50%.
But that is only the beginning.
