The efavirenz in LTE required fasting and to be taken at bedtime. That’s because food increased drug levels and made side effects worse.
Dolutegravir, the D in TLD doesn’t have these issues. It can be taken any time of the day, with or without food. The only restrictions are taking D with minerals like calcium, iron, or magnesium. The dolutegravir will bind to the mineral instead of the integrase enzyme in HIV, where it’s supposed to go. If your doctor has you taking a supplement, then take one in the am and the other in the pm so they’re not in your gut at the same time.
A few people who taked TLD at night report trouble sleeping. If that happens, simply switch to a morning dose.
Also, some people get stomach upset when taking any kind of pill (not just TLD) on an empty stomach. For a few people, their stomach thinks the pill is food, so it makes acid to digest it. But the pill passes quickly, leaving the stomach full of acid with no food to neutralize it. If that happens, simply take the pill after a meal or snack.
If you’ve switched from LTE to TLD, it’s good to know that LTE suppresses weight. When you switch, you loose that weight loss effect, so it’s possible to gain weight with the switch.
