Reasoning - risk of cardiac arrest very high for patient D and immediate management will only take a few minutes. B vs C is a tough call but I think B is more imminently life threatening.

@VipsMDMEd PE. Can present with crackles, wheezes, anxiety symptoms and mimics CHF/Pneumonia/URTI. Almost NEVER has an S1Q3TR on ECG and a diagnosis I've often seen dismissed without good reason.
