An observation: A hundred years ago, patients presented with signs and symptoms but had very limited diagnostic testing. Doctors made presumptive diagnoses using mostly history and exam, and autopsy later revealed whether they were correct. Those who consistently got it right became legends. Treatment options were minimal. The art was almost all in the diagnosis.

Today, patients often arrive with an established or obvious diagnosis (“I’m having a flare of my arthritis,�� “I’m here for follow-up of XYZ,” “The X-ray shows pneumonia”). We usually know what’s going on very quickly. The physician’s role is to ensure nothing important is missed, start treatments that can be complicated, and protect patients from unnecessary testing or interventions.

For the best modern doctors, diagnosis still matters, but just as important is helping patients navigate a complex system safely. A very different world from the past.
