UPDATE: Most Immigrants May Not Need to Leave the U.S. to Get Green Cards โ But Confusion Remains
Recent reports created concern among immigrant communities after a USCIS policy memo suggested that some individuals applying for permanent residency may need to leave the United States and continue the process from abroad, except in limited circumstances.
DHS has since clarified that this is not a blanket change and that many applicants may still be able to remain in the U.S. while their cases are reviewed. However, important details remain unclear, including who may be affected, how pending cases will be handled, and how individual determinations will be made.
For international medical graduates, physicians, residents, fellows, researchers, and healthcare workers, immigration clarity is important. Many are serving patients, supporting hospitals, advancing research, and contributing to communities across the country.

For years, I dreamed of bringing the organizations that shape the IMG journey into the same room, not just to speak about IMGs, but to speak with IMGs.
At @TheIMGevent 2026 in Miami, that dream became real.
We had the honor of bringing together leaders from the institutions that define so many parts of our path: Dr. Eric Holmboe and Dr. Catherine Apaloo from ECFMG/ @Intealth@ECFMG_IMG , Dr. Alex Mechaber from USMLE/NBME, Jeanette Calli from @TheNRMP /The Match, Dr. Humayun โHankโ Chaudhry from @TheFSMB , and other national leaders committed to medical education, licensing, certification, and the future of the physician workforce.
Having all of them together, in one place, in front of nearly 1,000 IMGs, residents, fellows, students, and physicians, was one of the most meaningful moments of my life.
Between panels, we also had a private meeting with some of the biggest figures in medicine to discuss the future of IMGs, the challenges our community faces, and how we can move forward together.
This started as a small meetup in Central Park in 2021.
Today, Project IMG represents more than 150,000 doctors and trainees around the world, with events across Miami, New York, Chicago, India, Pakistan, the Philippines, Cairo, Mexico, Morocco, and many cities across the United States.
None of this happened overnight. It took years of building relationships, earning trust, staying consistent, and believing that IMGs deserved a seat at the table.
To @ProjectImg team and The IMG Event team: thank you. This would not have been possible without your work, your heart, and your belief in this mission.
This event was more than a conference.
It was proof of what happens when IMGs come together with purpose, unity, and vision ๐ค๐ฉต๐๐ช

URGENT POLICY ALERT ๐จ
Project IMG is deeply concerned by the newly announced USCIS policy requiring certain temporary visa holders who seek permanent residency to leave the United States and apply from their home countries, except in extraordinary circumstances.
While immigration policy is complex, the healthcare impact must be considered urgently.
Thousands of international physicians are currently in the middle of critical transitions: completing residency, entering fellowship, finishing fellowship, obtaining waivers, starting attending jobs, and preparing to serve communities that already face severe physician shortages.
These are doctors already trained by U.S. hospitals, already caring for patients, and already part of the healthcare workforce. Forcing delays or interruptions in their immigration pathway could destabilize residency programs, fellowship programs, hospitals, clinics, and underserved communities across the country.
The United States is already facing a projected physician shortage. AAMC projects a shortage of up to 86,000 physicians by 2036, and HRSA projects a shortage of 141,160 physician full-time equivalents by 2038.
This policy could make an already fragile healthcare workforce crisis worse.
Physicians and physicians-in-training must be exempted.
Protect physicians.
Protect patients.
Protect access to care.
Project IMG calls on policymakers, healthcare institutions, medical organizations, and immigration authorities to urgently consider the consequences this could have on patient care and to ensure that doctors are not forced out of the system at the exact moment America needs them most.
