One of the limitations of therapeutic drug monitoring is timing, as clinicians often need to wait until trough to obtain data. For patients receiving ustekinumab that may mean waiting up to 8 weeks.

In a prospective IBD study, investigators evaluated whether clearance-based mid-cycle pharmacokinetic (PK) assessment could provide insights into disease control before the end of the dosing interval. Findings suggest optimal pharmacokinetic profiles were associated with higher clinical and biochemical remission rates.

❓Can optimizing intravenous infliximab support long-term treatment durability?

Data presented at the British Society of Gastroenterology (BSG) LIVE 2026 demonstrated significantly higher infliximab (IFX) persistence beyond 5 years with proactive therapeutic drug monitoring (TDM)-guided dose escalation compared with reactive dose escalation.

Long-term real-world data suggest that proactive TDM combined with IFX dose escalation may:
✔️ Improve pharmacokinetic exposure
✔️ Reduce anti-drug antibodies
✔️ Support greater long-term treatment persistence compared with reactive optimization

At approximately 70 months of follow-up, >80% of patients receiving proactive dose escalation remained on IFX vs >60% of those receiving reactive escalation (p=0.03).

When it comes to IBD, age isn’t just a number – it’s a clinical factor.

Older adults with inflammatory bowel disease (IBD) had 35% higher odds of developing antibodies to infliximab (IFX), even after controlling for dosing and IBD subtype. Notably, this population was also less likely to receive dose escalation compared to younger patients.

Increasing age was independently associated with antibody to infliximab (ATI) development, potentially putting older adults with IBD at risk for suboptimal treatment exposure.
