In Case You Missed It | At DDW 2026, new data showed that validated serological signatures can accurately identify inflammatory bowel disease (IBD) and distinguish Crohn’s disease (CD) from ulcerative colitis (UC) – in both adult and pediatric populations.

Using a panel of anti‑microbial and autoantigen antibodies, researchers trained and validated models that demonstrated:
✅ Strong performance differentiating IBD vs non‑IBD (AUC ~0.85 in adults; 0.80 in pediatrics)
✅ High accuracy in stratifying UC vs CD, with especially robust results in pediatric cohorts (AUC up to 0.96)
✅ Clear serologic patterns: anti‑microbial antibodies supporting CD classification, while pANCA/PR3/αvβ6 signatures were enriched in UC and colonic CD

These findings highlight the potential of serologic profiling to enable earlier diagnosis, improve disease classification and support future risk stratification.

Guided by translational research presented at DDW® 2026, we are developing a next‑generation blood‑based test to better support identification and longitudinal assessment of treatment‑eligible MASH patients in a rapidly evolving care landscape.

Often asymptomatic, MASH affects up to 20 million people in the United States, with prevalence rising alongside obesity and type 2 diabetes. Providing a noninvasive blood-based solution will help enable these patients to be identified, often more timely and cost-effectively than biopsy or resource intensive-imaging, so treatment can be initiated before MASH progresses to cirrhosis.

In Case You Missed It | Anti-PR3 may predict primary non-response to anti-TNFs

A small study recently found that ⅔ of patients that were positive for anti-PR3 antibodies experienced primary non-response to anti-TNFs, compared to only ¼ of patients that were negative for anti-PR3.

Roughly 30 years ago, pANCA was introduced as a serological biomarker for ulcerative colitis (UC), a cornerstone in non-invasive UC detection.

Fast-forward to this past December: We launched IBD Precis with the first update to serological testing for UC since pANCA.

This 5thgeneration IBD diagnostic panel introduced anti‑integrin (anti‑αvβ6), a novel UC marker that has demonstrated 79% sensitivity and a 94% specificity in differentiating UC from non-IBD.

Prometheus Laboratories is the only clinical laboratory in the United States to offer this marker, available as part of our IBD Precis diagnostic test.

Did You Know | Serology can help better classify IBD phenotypes.

Data demonstrating how serologic patterns can help stratify ulcerative colitis vs Crohn’s disease in routine clinical practice was presented at ECCO.

Validation of UC vs. CD discrimination showed:
· Adult patients: 91% sensitivity and 73% specificity (AUC=0.87)
· Pediatric patients: 93% sensitivity and 83% specificity (AUC=0.96)

Understanding serologic signatures offers another precision medicine tool to accurately phenotype IBD patients for more informed clinical decision-making.
