Last night, the triage phone rang and the ambulance service informed us that they were bringing in an 89-year-old patient with a FAST-positive result. Immediate preparations were made to receive the patient, and the stroke team was alerted and attended promptly in anticipation of arrival.
On arrival, the patient was assessed and confirmed to be experiencing a stroke based on the presenting symptoms. The patient was immediately transferred for an urgent CT scan after the radiology department was informed. Imaging findings were consistent with a transient ischaemic attack (TIA).
The doctor and stroke team explained to the patient’s relatives that thrombolysis was required to prevent further brain damage. The benefits and potential side effects of the medication were clearly explained. Consent was obtained from the relatives.
The patient was weighed using the bed scale, thrombolysis was prescribed, and alteplase was administered as per protocol. The patient’s condition began to improve before the infusion was completed.
Following treatment, the patient was transferred to the stroke unit for ongoing management.
This is how multidisciplinary teamwork work in healthcare, where everyone understands their role, acts promptly, and recognises that life is precious and that negligence can have serious consequences.
In Nigeria, patient and their relatives lose their loved ones because they have to walk down to pharmacy to pay for a critical medication before it can be administered. Most times the medication will be out of stock, I reflect on these things and i wept for my country.
