State of health facilities across the country can no longer be a quiet issue‼️
This is the state of Balogun community PHC, in Ikire Irewole LGA, Osun state. The only facility serving a community of over 6000 residents.
No maintenance has been done on the facility since its construction in 2005. Now the roofing sheets have blown away, windows and doors collapsed, the walls are cracked and the environment is surrounded with bush.
Community members have to travel over 5km to a neighbouring community to get basic treatment and during emergencies, chances are that travel time will make things worse for patients.
The community head told us that several letters have been written to the authorities (Senator, Federal Rep and LGA chairman) about the condition of this facility but they received no positive response.
We implore the office of the Gov @AAdeleke_01, the @NphcdaNG, @Fmohnigeria, to facilitate the reconstruction of this facility and supply of medical equipment for standard care.

🗣️“When we say check your state budget, this is exactly what we are talking about.”
In 2020, we visited Government Day Secondary School, Diko, Gurara LGA, Niger State, and found the school in a completely dysfunctional and dilapidated state. Meanwhile, this is the only government secondary school serving about 14 different communities, yet students are forced to learn in terrible conditions.
Classrooms have no doors or windows, roofs are collapsing, ceilings ripped off, and many students sit on bare floors during lessons. Even more concerning, the schools lack perimeter fencing, exposing students and staff to serious security threats at a time insecurity remains a major national concern.
In 2025, the school appeared in the Niger State Budget with N457.6m allocated for renovation. According to her 2025 fourth quarter Budget Implementation Report, N400m i.e. 87.4% of the funds has already been disbursed.
But the shocking reality is that there is absolutely no evidence of project execution on ground. No contractor. No ongoing work. No rehabilitation. Nothing.
We visited both the senior secondary school and the relocated junior secondary section over 3km away, and the conditions remain the same, abandoned, unsafe, and neglected.
🧐 How can N400m be reportedly spent while children continue learning in collapsing classrooms without furniture or security?
🧐 Who received the funds?
🧐 Who certified the project performance?
🧐 Where exactly was the money spent?
Sadly, this is gradually becoming a disturbing trend across several states. Despite increasing allocations and rising public revenues, citizens continue to see little or no corresponding development in critical sectors like education, healthcare, and infrastructure.
We call on the @officialEFCC and @icpcnigeria to urgently investigate this matter and ensure accountability for public funds meant for education development.
Nigeria should not continue budgeting for progress while communities remain trapped in neglect. We must do better.

Yesterday, we visited the site of the “construction and furnishing of an ultra-modern conference and e-learning facility at Bende Secondary Grammar School, Bende, Abia State” captured in the 2024 FG budget.
Findings from https://t.co/1b9IJVu1Oe show that N265.3m has already been paid to Muslac Techno Company Limited since December 2025.
However, the school informed us that they are completely unaware of the project. More troubling is the fact that despite the school having sufficient available land, the project was seen diverted from the original beneficiary to another community in Ọnụ Inyang, at an isolated location along the Bende-Ohafia express road.
At the time of our visit, no workers were found on site. The construction site already appears abandoned, with grasses gradually overtaking the structure on ground. For a project that has received such substantial payment for over six months, the level of work visible raises serious concerns about implementation, supervision, and value for public funds.
Kindly call on the @FmstNg, the implementing agency, and anti-graft agencies @officialEFCC and @icpcnigeria to investigate this project and provide the public with clear answers.

- What exactly justifies the N265.3m already paid?
Public projects must serve the communities they were budgeted for, not become another case of questionable execution and weak accountability.

Ruwaza community, a rural settlement with about 4,500 residents, is currently facing a severe lack of access to clean water.
Women and children have to trek long distances to the only available source of water which is an almost dried up hole.
The community’s woman leader, Mrs. Anatu, expressed deep concern over the situation; “Our children have suffered from the lack of water. Even the ground does not bring water well. We have to line up and queue for a long time before we can get water for use at home.”
Another woman in the community shared how her child fell ill because of how unsafe the water is.
This is not an uncommon experience for most residents. They are at risk every time they use this water.
Beyond water challenges, Ruwaza community is also lacking in critical infrastructure. The community has no healthcare facility, no electricity supply, and while the community has a primary school, children who wish to continue to junior or senior secondary education must travel at least 17 km to neighboring communities.
This long distance discourages many families from sending their children, particularly girls, to school.
We implore the offices of @GovWike, Hon. Danladi chiya, Hon. Abdulrahman Ajiya to address the needs of this community.

For years, the people of Oversea Community in Nasarawa had no real health centre to rely on. Just a small room that was cramped and unfit for health care and almost no medical personnel to administer care.
Over 11,000 residents, especially women and children, were being treated under these conditions until our first visited in 2020.
Our series of advocacy plus the relentless efforts of community representatives and the WDC led to the construction of a proper PHC facility in 2025.

Today, we celebrate every worker whose daily efforts keep our nation moving, from teachers, farmers, artisans, civil servants, traders, engineers, drivers, security personnel, cleaners, entrepreneurs, and countless others whose labour sustains our communities.
There is no better way to mark this day than with a win for public service delivery. This month, the @KogiStateGov through its Primary Health Care Development Board, in collaboration with the Ajaokuta LGA Health Department, deployed three health personnel to Gadumo PHC after the facility remained under lock and key for about four years due to lack of staff.
This progress followed sustained advocacy by us and the community through formal letters, citizen engagement, and consistent follow-up.
A public facility without workers cannot serve the people. A completed project without personnel is only a structure. Today, Gadumo PHC moves closer to becoming what it was meant to be, a place of care, hope, and service for over 10,000 residents.
As we celebrate International Workers' Day today, we also call for stronger institutions, adequate staffing of public facilities, and continued responsiveness to citizens’ voices.
When institutions listen and workers are empowered, communities win.

Pregnant women in Chatti community share stream with Cattles and Reptiles‼️

Chatti community is in Kuje Area Council, FCT and for over 100 years, they wake up and head to a muddy contaminated stream shared with animals to get water.
This is the only source of water for over 7,500 people; young, old, women, children.
Mrs. James, a pregnant woman, shared how she has to get to the stream by 5am to get water for her family. Water that is often polluted by cattle. Water that is unsafe, but she has no options.
In Chatti, the absence of clean water is only the beginning. No functional health facility. No school. No basic infrastructure.
We implore your office and the @FMWRNigeria, to facilitate the provision of clean water and other amenities for Chatti community.

For those who have asked what we meant by fraudulently delivered projects in our last report, the Kugbo Bus Terminal is yet another example.
The terminal was officially commissioned in June 2025, amid promises that it would modernize transportation in Abuja, improve commuter safety, and help address the menace of 'one-chance' transport crimes. It was presented as a major step toward improving public transport infrastructure in the Federal Capital Territory.
However, despite the public fanfare that accompanied its commissioning, the facility remained unused and non operational for more than nine months after it was unveiled. A public project commissioned in June 2025 should have immediately begun serving the commuting public. Instead, the terminal sat idle for months without fulfilling the purpose for which it was built.
This raises a fundamental question about project delivery and accountability. Why commission a public facility that is not ready for use? Commissioning projects that are not operational misleads citizens and creates the false impression that infrastructure is being delivered when, in reality, the public cannot access or benefit from it.
With parts of the terminal’s structure being damaged after a short rainfall yesterday further deepen concerns about the quality of construction and oversight. Public infrastructure funded with taxpayers’ money should be durable, functional, and able to withstand normal environmental conditions.
Citizens deserve clear answers.
Who built the terminal?
Who supervised the project?
Who inspected and certified it before commissioning?
Why was it commissioned before it was ready for public use?
Public infrastructure must go beyond ribbon cutting ceremonies. Projects should only be commissioned when they are fully ready, functional, and able to serve citizens immediately.
At tracka will continue to demand transparency, quality, and accountability in public projects, because Nigerians deserve infrastructure that works and public resources must never be wasted.

Tracka’s first visit to Tukurawa community, Pai Ward in Kwali FCT, showed a community barely getting by.
Over 200 children were learning under open roofs and on the floor.
We held a townhall meeting with them, wrote letters to their LGA chairman and other relevant agencies.
After repeated advocacy efforts, the school was finally reconstructed with enough furniture for children to learn with.

Gongola Dogo Nini Ward, Patiskum LGA of Yobe State have been asking for basic amenities for over a decade.
The residents have tried to use their personal funds to fix boreholes, electricity, etc and written to the local government for help but nothing has been done.
They have been frustrated at the system that has left them with no working roads, no markets, no water, no school!
We implore your office and the @YobeStateGovt, to provide this community with basic infrastructure for better quality of life.
An accident occurred yesterday at Naira & Kobo Junction. Our rescue team responded promptly, administered first aid, and successfully transferred the victim to the hospital for further treatment.

In February 2026, we paid monitoring visits to several PHCs across Osun State.
Oke Otubu, Oke Bola Modakeke, and Oloyede PHCs are currently in poor and dilapidated conditions despite their years of existence.
These communities have repeatedly called for local government interventions but till date, nothing has been done to imporve care across these facilities.
There are no medical equipment, proper infrastructure or sanitation facilities, and residents continue to use these facilities as their only source of medical care.
We implore the @NphcdaNG, @Fmohnigeria, and the office of Gov @AAdeleke_01, @Osun_State_Gov to prioritize the reconstruction of these facilities for residents to access quality healthcare.

Ededimeji and Akoda PHCs have benefited from recent revitalization efforts. Both facilities have undergone structural repairs and improvements including repainting, borehole installation, building repairs etc.
This has led to better service delivery and increased patient turnout.
As each part of LEA Primary Sch collapses, the future of 3,600 children hangs in the balance⚖️
At LEA Primary School, Yangoji in Kwali Area Council, FCT, over 3600 pupils are learning under dangerously deteriorating buidlings.
For over 2 years, the school building has been left with damaged roofs, broken classrooms, and unsafe learning spaces. Half the time, pupils are scared of the building collapsing on them.
This is a community of about 9,500 residents, yet this is the only accessible public primary school.
The collapse of one of the blocks has forced pupils from pry 1 and 2 to share the same space, leading to severe overcrowding; over 600 pupils in each class.
We implore the offices of Hon. Danladi Chiya, Hon. Abdulrahman Ajiya, Gov @GovWike, Sen @IretiKingibe and the @NigEducation to facilitate the reconstruction of this facility and provide learning facilities for the children of Yangoji.
